Comprehensive Exam Form

This downloadable form is to be used ONLY at the completion of the oral comprehensive exam and ONLY if the written comprehensive exam has already been successfully completed.  Thus, the Comprehensive Exam is two parts, written and oral. 

This form is to be completed ONLY when both parts have been successfully passed.
